Expert Management of a Rare and Devastating Abdominal Trauma at Bombay Gastro & Cancer Institute, Bathinda

A 32-year-old male was brought to the emergency department after sustaining a high-voltage electrical injury followed by a fall from an electric pole. He arrived in a critical condition and required immediate resuscitation.

Clinical examination revealed gangrene of the left hand, a severe traumatic injury to the anterior abdominal wall, and evisceration of multiple loops of small intestine. The patient had a complete transection of the jejunum with active intra-abdominal bleeding, making it an immediately life-threatening surgical emergency.

Emergency Surgical Management

The patient was taken directly to the operating theatre, where Dr. Raman Garg, Consultant Surgical Gastroenterologist, Advanced GI, Laparoscopic & HPB Surgeon at Bombay Gastro & Cancer Institute, Bathinda, performed an emergency exploratory laparotomy.

Complete transection of the jejunum identified during emergency exploratory laparotomy

During surgery, a completely transected segment of the jejunum was identified. After careful assessment of bowel viability, the damaged segment was resected, and a primary end-to-end jejunojejunal anastomosis was performed to restore intestinal continuity. The traumatic abdominal wall injury was meticulously repaired after achieving complete haemostasis and thorough abdominal lavage.

The operation successfully controlled the life-threatening injuries and restored the integrity of the gastrointestinal tract.

Postoperative Recovery

Despite the severity of the trauma, the patient made an excellent recovery.

  • Early return of bowel function.
  • Gradual progression to oral feeding.
  • No major postoperative complications.
  • Discharged in satisfactory condition on the 5th postoperative day.

Why This Case Is Important

High-voltage electrical injuries associated with major abdominal trauma are exceptionally rare and often involve multiple organ systems. Complete jejunal transection with bowel evisceration following electrical injury and a fall is an extremely uncommon surgical emergency associated with a high risk of mortality.

Successful management requires rapid resuscitation, timely exploratory laparotomy, meticulous bowel reconstruction, control of contamination and bleeding, and comprehensive postoperative care.

This case highlights Dr. Raman Garg's expertise in managing complex abdominal trauma, intestinal injuries, bowel perforations, emergency gastrointestinal surgery, and advanced reconstructive procedures, resulting in an excellent clinical outcome despite the severity of the injury.
